Please select date and time for your appointment. Pre-Screening Questions: -Are you currently pregnant? -Do you have Keloid skin? -Are you a Diabetic? If so, is it controlled (note from Physician required)? If you answered "yes" to any of the above questions, you are not a candidate for Permanent Cosmetics. Please do not take any Aspirin/Aspirin-related products before your appointment. Also, do not drink coffee before your appointment. Clients using prescription medication for Acne such as, Acutane, and/or clients who have went through Chemotherapy, you must wait a year before Permanent Makeup. If you are using any facial scrubs or products containing AHA (Alphahydroxy or Betahydroxy), please do not use within 72 hours of your appontment. Cancellation Policy

For cancellations, please contact Dezaree' Lusk (469)499-6200 48 hours prior to your appointment. Your deposit will not be refunded for any missed appointments. You do not need to pay another deposit, if you are just rescheduling. Please send your deposit through PayPal to divinehaircomp@gmail.com. Lastly, please be on time to your appointment.
